Nicholas Brendon, renowned for his role as Xander Harris in the iconic series *Buffy the Vampire Slayer*, passed away at the age of 54 on March 20, 2026. His family reported that he “passed in his sleep of natural causes,” and details surrounding his death have now come to light.

A death certificate revealed that Brendon’s demise was attributed to heart disease, specifically caused by blocked arteries and high blood pressure. Additionally, he had suffered from pneumonia. A previous heart attack was noted as a contributing factor to his condition.
Medical Findings
According to Putnam County Coroner Todd Zeiner, the autopsy indicated several serious health issues:
- Markedly enlarged heart
- Severe stenosis of the right coronary artery
- Moderate stenosis of the left anterior descending artery
- Moderate stenosis of the left circumflex artery
- Acute pneumonia
- Inflammation of the small bowel
This inflammation was likely linked to ischemic changes resulting from cardiogenic shock.
Brendon’s Health Struggles
Brendon had been transparent about his health challenges. In August 2022, he shared a post on Instagram where he detailed a hospitalization due to tachycardia, an abnormal heart rhythm characterized by a resting heart rate exceeding 100 beats per minute. Normal resting rates typically range from 60 to 100 beats.
His manager, Theresa Fortier, noted that he faced similar heart issues following a second spinal surgery for cauda equina syndrome. This condition results from damage to nerve roots at the lower spinal cord. Brendon first sustained a spinal injury in February 2021 from a fall, which necessitated emergency surgery.
Complications from his second spinal surgery in October 2021 led to tachycardia, prompting further medical attention. After this incident, Brendon was diagnosed with a congenital heart defect.

Brendon’s passing has prompted heartfelt tributes from former co-stars. Sarah Michelle Gellar, who portrayed Buffy, honored him with a touching message, saying, “I saw you, Nicky. I know you are at peace, in that big rocking chair in the sky.”
David Boreanaz, his on-screen companion, stated, “There are people you work with and then there are people you share time with. Nick was the latter.” Alyson Hannigan, who played Willow, remembered him fondly, saying, “My Sweet Nicky, thank you for years of laughter, love, and Dodgers.
