Coroner Urges Squad to Compel DPJ Cooperation
A recent report by Coroner Julie-Kim Godin shed light on the tragic case of a 16-year-old boy whose struggles with mental health and lack of support ultimately led to his untimely death. The coroner’s findings highlighted severe shortcomings in the intervention processes by the Directorate for Youth Protection (DPJ) in Quebec.
Key Findings of the Coroner’s Report
The coroner’s investigation outlined a concerning trajectory from 2019 to 2024, marked by increasing vulnerabilities and red flags. Despite numerous reports made to the DPJ, the boy’s case was often overlooked.
- The boy stopped attending school for almost four years.
- He lived in a vulnerable familial environment and experienced severe mental health issues.
- Reports were made to the DPJ due to concerns regarding his isolation and overall well-being.
In September 2021, various professionals expressed their concerns, indicating a need for DPJ’s intervention. However, DPJ determined that the boy had access to mental health services and did not retain the report. A single report was acknowledged in September 2022, but actions taken were minimal.
Lack of Effective Intervention
In March 2024, another report was acknowledged but subsequently withdrawn by the DPJ due to staff changes. Tragically, one month later, the boy was living in a hotel room. The coroner emphasized the disconnect between well-meaning health professionals and the DPJ’s inaction, which created barriers to necessary support.

- No structured intervention plans were established despite ongoing concerns.
Recommendations for Improvement
In response to this alarming scenario, Coroner Godin called for systemic changes. She recommended the creation of an independent crisis intervention team specialized in youth issues. This team would aim to enhance the coordination and efficacy of interventions when traditional pathways fail.
